The 4th house is the private room. Where you are from, what you are like at home, and the version of you that is not performing for anybody.
An overlay here is the reason some people feel like family within a month. The other person is standing in the foundations rather than in the front room, and the feeling is usually described as familiar rather than exciting.
Familiar is not the same as safe. The 4th also holds whatever your family taught you, so a planet arriving here can land on warmth and on an old bruise in the same movement.

When the two directions are uneven, one person often describes the relationship as home and the other describes it as something they visit. Neither description is wrong, and saying so out loud saves a great deal of guessing.
What makes an overlay here worth reading
The 4th cusp is the IC, one of the four angles, so a planet within a few degrees of it is doing considerably more than one drifting near the far end of the house.
Three or more personal planets in your 4th is a cluster, and it is the signature Umbriel names in a pair reading. It usually shows up in ordinary life as somebody who has a key.
What it does not tell you
The angles need a real birth time. Four minutes of error moves the IC by a degree, and an hour of error can move a planet from your 4th to your 3rd or 5th, which are three unrelated readings.
And a strong 4th house overlay describes closeness, not health. A person can be deeply familiar and not good for you, and the chart will not make that distinction for you. Only living with it does.
